Acknowledgement

Acknowledge that:

  • MagnaWave/Pulsed Electromagnetic Fields (PEMF) is NOT to be used to diagnose, treat, cure, or prevent any disease or medical condition.

  • The session is considered complementary and does not replace standard veterinary/doctor care, advice, or treatment from a licensed veterinarian/doctor.

  • Be aware that there may be potential risks associated with a PEMF session for myself or my animal, including but not limited to restlessness, discomfort, or temporary worsening of symptoms.

  • Understand the effects of PEMF may vary from animal to animal, person to person and there is no guarantee of results.

Release of Liability

Hereby release, waive, and discharge Double M Performance LLC, its employees, agents, and representatives, from any and all liability, claims, or demands arising from the use of MagnaWave/PEMF on yourself or your animal, including but not limited to:

  • Any injury, harm, or side effect your animal or you may experience as a result of using MagnaWave/PEMF;

  • Any loss or damage to your animal, you or any personal property during or after use of MagnaWave/PEMF;

  • Any financial, medical, or veterinary/doctor issues resulting from the use of MagnaWave/PEMF.

*PEMF can drain batteries or interfere with battery operated devices.

Agree to indemnify and hold harmless Double M Performance LLC, Magnawave, its staff, and representatives from any claims, liabilities, damages, or expenses that may arise due to the use of MagnaWave/PEMF on you or your animal.

Voluntary Participation

Understand that the use of MagnaWave/PEMF on yourself or your animal is entirely voluntary, and you can discontinue sessions at any time.  Acknowledge that you have had the opportunity to consult with a licensed veterinarian/doctor before participating.

Payment

Payment is due before a session starts! (Unless otherwise talked about)
